Qualifications

• Bachelor’s degree in social work, public health, or 2+ years providing outreach and case management to high-risk populations.

Responsibilities

• Outreach and connect people to shelter, housing, and wrap-around services to support housing stability.
• Maintain 20-25 active client caseload with intake, assessments, and case notes.
• Document data and generate daily, weekly, and monthly activity reports.
