Responsibilities include admissions, ongoing assessments, and coordination with interdisciplinary teams. A valid RN license is required; prior LTAC/skilled nursing experience is valued.

Qualifications

• Active RN license in good standing in the state of practice.
• Experience in a skilled nursing or long-term care setting is preferred but not required—the organization will invest in the right heart and mindset.

Responsibilities

• Supervise medications and treatments and oversee RN/LPN charge nurses, CNAs, and medication aides.
• Complete required admission and routine resident assessments.
• Monitor resident conditions and respond quickly to changes, ensuring safety and comfort.
• Support activities of daily living.
• Maintain clear, timely documentation.
• Communicate with interdisciplinary teams and families.
• Adhere to infection control and safety standards.
• Perform other duties as assigned.
