Qualifications

Responsibilities

• Collects patient health data with patient/family involvement.
• Performs initial assessments and documents nursing component.
• Adapts nursing procedures to patient needs and age groups.
• Develops and updates the care plan and goals.
• Coordinates discharge planning as part of admission process.
