Quality Home Health, Hospice & Palliative Care in Utah County seeks a compassionate RN for Night On-Call. This full-time role provides after-hours triage, urgent nursing visits, and emergent admissions in a field-based hospice setting.

The ideal candidate has an active Utah RN license, hospice experience, and excels in independent decision-making with strong communication skills to support patients and families during end-of-life care.

Competenze

• Current, active Registered Nurse (RN) license in Utah.
• Experience in hospice, home health, or palliative care preferred.
• Strong clinical assessment and symptom management skills.
• Excellent communication, critical-thinking, and problem-solving abilities.
• Ability to work independently in urgent after-hours situations.

Mansioni

• Respond promptly to after-hours patient and caregiver calls.
• Assess patient needs over the phone and determine when an in-person nursing visit is necessary.
• Provide clinical guidance and education on symptom management and comfort measures.
• Perform urgent nursing visits for uncontrolled symptoms or changes in condition.
• Coordinate with hospice physicians and leadership when additional orders are needed.
• Provide education and support to patients and families during end-of-life transitions.
• Complete emergent hospice admissions when needed during on-call hours.
• Maintain accurate documentation in the EMR.
