Job summary

Dignity Home Health & Hospice is seeking an On-Call Registered Nurse (RN) to provide after-hours clinical support to patients and families. You will perform assessments, education, and hands-on nursing care in homes, assisted living, and skilled facilities with weekend/overnight shifts available.

Ideal candidates have 1 year of home health/hospice experience, strong clinical judgment, compassionate communication, and solid documentation.

Qualifications

• Minimum 1 year in home health or hospice preferred.
• Current Utah RN license required.
• Current CPR certification required.
• Valid Utah driver's license and auto insurance.
• Experience with EMR and healthcare tech encouraged.

Responsibilities

• Respond to patient needs outside regular hours via phone triage and visits.
• Perform comprehensive nursing assessments in homes, assisted living, and SNFs.
• Provide education, reassurance, and support to patients and families.
• Complete admissions during after-hours when necessary.
• Maintain accurate, timely clinical documentation.
• Coordinate with interdisciplinary team to ensure continuity of care.
