Job summary

Guardian Angels Care Center – Elk River, MN is seeking an LPN or RN for the Transitional Care Unit (400 Wing). The role covers 10:30 pm to 7 am on a part-time basis, with 56 hours per two-week pay period and every other weekend plus holidays.

You will assess and monitor residents, notify physicians and families of changes, and supervise nursing staff to follow the plan of care and facility policies. Licensed professionals must be Minnesota-licensed with CPR, and are expected to maintain

Qualifications

• Currently licensed as an LPN or RN in Minnesota.
• Current CPR certification is required.
• Prior experience is preferred.
• Must be at least 18 years of age.
• Ability to maintain confidentiality at all times.
• Self-motivated, ability to prioritize.

Responsibilities

• Provides dignified, resident-centered care under the supervision of a licensed nurse.
• Responsible for assessment, care and services provided to assigned residents within shift worked.
• Supervises and monitors assigned staff to ensure compliance with resident plan of care and facility policy and procedures.
• Follows universal precautions, infection-control guidelines, sanitation, and safety standards of practice.
• Upholds confidential information as required by policy related to HIPAA.
• All duties performed in accordance with current standards, guidelines and regulations.
