Job summary

Oak Hill is seeking a Direct Support Professional to assist individuals with daily living, health monitoring, and community participation in Monroe, CT. The role includes night shifts on Fridays and Saturdays, with a base rate of $20.39 per hour and potential differential pay for specific schedules.

You will support personal care, medication administration, nutrition, mobility, and documentation in the EHR, while following health plans and safety protocols.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional need to stay beyond a scheduled shift.

Responsibilities

• Assist with daily living activities, including personal care, grooming, hygiene, and independence.
• Monitor health information and administer medications as trained.
• Provide transportation to appointments and activities.
• Prepare meals and support during meals according to dietary needs.
• Assist with mobility and transfers using approved equipment.
• Document daily activities, health information, and medication administration in the EHR.
• Follow emergency procedures and safety protocols; perform First Aid/CPR when required.
• Collaborate with team and participate in meetings and trainings.

Education
