Qualifications

Responsibilities

• Assesses patient needs and plans, implements, evaluates and documents nursing care.
• Formulates a plan of care including discharge planning.
• Implements the plan of care with supervision and coordination of the healthcare team.
• Promotes continuity of care through effective communication with care providers.
• Documents assessments, planning, interventions and patient response in the medical record.
• Provides care according to best evidence and participates in shared leadership and education.
