Job summary

Texas Children's Hospital is seeking a Care Coordinator Assistant to provide non-clinical administrative support for the care coordination process, including discharge planning and utilization management.

This role supports communication with internal and external contacts, assists with transition planning, maintains data integrity, and helps ensure timely, patient‑ and family‑centered care in a busy hospital environment.

Qualifications

• HS Diploma or GED
• Some college coursework preferred
• 2 years' experience in hospital care coordination/case management, home health, quality improvement or a related field

Responsibilities

• Provides department-related and other appropriate communications to internal and external customers.
• Assists utilization management processes to maintain compliance with federal, state and managed care guidelines.
• Assists transition/discharge planning and care coordination processes to transition plans and enhance patient/family satisfaction.
• Maintains data integrity.
• Provides professional, efficient and courteous service while handling confidential information.
• Collaborates with peers, nursing and medical staff; responds to requests promptly and tactfully.
• Obtains information required for response and relays messages to staff, customers, and providers.
• Promotes a positive work environment and contributes to the overall team effort.
