Qualifications

Responsibilities

• Provides department-related and other appropriate communications to internal and external customers.
• Assists utilization management processes to maintain compliance with federal, state and managed care guidelines.
• Assists transition/discharge planning and care coordination processes to transition plans and enhance patient/family satisfaction.
• Maintains data integrity.
• Provides professional, efficient and courteous service to the customer while maintaining confidentiality.
• Works with peers, nursing and medical staff and maintains a positive attitude; responds to requests tactfully and promptly.
• Obtains information required for response and communicates pertinent information to staff, customers, and providers; relays messages efficiently.
• Promotes a positive work environment and contributes to the overall team effort.
