Montello Care Center in Montello, WI is seeking a licensed Practical Nurse (LPN) to deliver direct nursing care to residents under supervision of a registered nurse and per care plans. You will participate in assessments, care planning, and coordination with the health care team.

Job Summary:
TheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsibleparty, and all health team members.

Key Responsibilities:
Provide safe, effective nursing care per care plans and physician orders
Administer medications and treatments accurately
Assess residents and report changes promptly
Document care in the EMR
Monitors performance and mentor CNAs
Ensure infection control and safety compliance
Completes all other duties as assigned.

Qualifications:
Required:
ActiveLPN licenseinstate of Wisconsin orlicensedin any U.S state and be eligible for Wisconsin licensure by reciprocity.(in good standing)
CPR certified
Ability to meet physical and job‑related requirements
Must be English literate andpossessexcellent writtenandverbal communication skills
Preferred:
Long‑term care or skilled nursing experience
Clinical experience
