Holton Manor in Elkhorn, WI is seeking a Licensed Practical Nurse (LPN) to provide bedside nursing care in a on-site, long-term care setting. The ideal candidate will be clinically competent, licensed, and able to work with a multidisciplinary team to support residents’ well-being.

The role offers opportunities across statuses (Full-Time / Part-Time / PRN) with shifts including PMs, NOC, and weekends. Comprehensive benefits and ongoing education support are provided to eligible staff.

Qualifications

• Active LPN license in Wisconsin or eligible for licensure by reciprocity.
• CPR certification.
• Ability to meet physical and job-related requirements.
• Must be fluent in English with excellent written and verbal communication skills.
• Preferred: experience in long-term care or skilled nursing; clinical experience.

Responsibilities

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.
• Monitor performance and mentor CNAs.
• Ensure infection control and safety compliance.
• Complete all other duties as assigned.

Job Summary:

TheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsibleparty, and all health team members.
