Qualifications

• Experience with electronic health records and clinical workflows.
• Strong written and verbal communication skills.
• Ability to manage projects and coordinate with teams.

Responsibilities

• Implement products and execute ACO initiatives with guidance from Market Leadership.
• Support wellness visits, transitions of care, and care management initiatives.
• Train office staff and physicians on processes and technologies.
• Identify training needs and connect with leadership to drive improvements.
• Build relationships with physicians, providers, and health system executives to improve performance metrics.
