Holton Manor in Elkhorn, WI is seeking a Medication Assistant to work under a licensed nurse, administering and documenting selected medications, measuring vital signs, and ensuring resident safety in an on-site setting.

You will follow the Five Rights, maintain confidential records, communicate with residents and staff, and participate in ongoing training and tuition reimbursement. Competitive hourly wage with weekend options and comprehensive benefits.

Qualifications

• Current CNA certification in Wisconsin.
• Completion of a Wisconsin-approved Medication Aide/Medication Assistant training program.
• CPR certification required.
• Ability to perform blood glucose testing.
• Strong accuracy, detail orientation and resident safety focus.

Responsibilities

• Administer and document medications as delegated by a licensed nurse.
• Observe residents for effects, side effects and report concerns promptly.
• Measure and record vital signs including blood pressure, pulse and glucose levels.
• Maintain accurate, confidential documentation per policy and state regulations.
• Follow Five Rights and other medication procedures.
• Keep the medication/treatment area clean and organized.
• Communicate with residents, families and the care team.
• Assist with resident care needs within certification and training scope.

Job Summary:

Under the supervision and direction of a licensed nurse, the Medication Assistantis responsible foradministering and accurately documenting selected scheduled medications, including oral, nasal, transdermal, rectal, inhalant, and ophthalmic medications, to residents. The Medication Assistant is trained to measure and record vital signs, including blood pressure, radial pulse, and blood glucose levels via finger stick.

Compensation:

CNAMed Techbase hourly wage:$23.00 - $26.97per hourdependenton experience

Weekend Warrior Program – choose one of the following 3 levels:

Select Level: base hourly wage +$5.00/hour (every other weekend)

Pro Level: base hourly wage +$7.00/hour (3 weekends per month)

PLUSshift differentials- $2.00 per hour PMshiftand $2.50 per hour NOC shift

Key Responsibilities:

Administer and document approved medications as delegated by a licensed nurse andpermittedunder Wisconsin law

Observe residents for medication effects, side effects, and adverse reactions and promptly report concerns to licensed nursing staff

Measure and accurately record vital signs, including blood pressure, pulse, and blood glucose levels via finger stick

Maintainaccurate,timely, and confidential documentationin accordance withfacility policy and state regulations

Follow all medication administration procedures, including the “Five Rights” of medication administration

Maintain a clean, safe, and organized medication and treatment area

Adhere to all facility policies, procedures, and Wisconsin Department of Health Services regulations

Communicate effectively with residents, families, and interdisciplinary team members

Provide assistancewith resident care needs as assigned, within the scope of certification and training.

Qualifications:

Current Certified Nursing Assistant (CNA) certification in the State of Wisconsin

Completion of a Wisconsin-approved Medication Aide/Medication Assistant training program

Demonstrated competency in medication administration and related tasks as required by Wisconsin regulations

Ability to perform blood glucose testing via finger stick

Current CPR certification (or ability to obtain upon hire)

Ability to follow written and verbal instructions andmaintainaccuraterecords

Strong attention to detail, professionalism, and commitment to resident safety

Must meet all health, background check, and training requirements as required by Wisconsin law
