Job summary

Holton Manor in Elkhorn, WI is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care under supervision. You will administer medications, assess residents, document care, and support CNAs while maintaining safety and quality standards.

Responsibilities

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.
• Monitors performance and mentor CNAs.
• Ensure infection control and safety compliance.
• Completes all other duties as assigned.

Job Summary:

TheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsibleparty, and all health team members.

Compensation:

PLUS $2.00 per hour PM and $2.50 per hour NOC shift differentials
