Edgerton Care Center, Inc. is seeking a dedicated Certified Nursing Assistant (CNA) to provide patient/resident care under the direct supervision of licensed nursing staff. The CNA completes assigned tasks to ensure timely, efficient and safe care, with clear communication to patients, residents, families and other health team members.

• Must be in good standing on the State Nursing Assistant registry or appropriate entity.
• Ability to meet physical and job-related requirements.
• Must be English literate with excellent written and verbal communication skills.

Responsibilities

• Performs initial and ongoing data collection on all patients/residents under RN/LPN direction.
• Implements plan of care developed by the RN based on patient needs.
• Provides direct patient/resident care and reports changes to RN/LPN.
• Demonstrates time management and prioritization of tasks.
• Documents results of patient/resident care within policies and procedures.
• Addresses nutritional needs of patients/residents.
• Maintains effective communication with residents and families.

Job Summary:

Key Responsibilities:

• Performs initial and ongoing data collection on all patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
• Implementsplan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural,ethnicand religious/spiritual needs.
• Provides direct patient/resident care andreportscondition and changes to RN or LPN.
• Demonstrates time management and prioritization of tasks.
• Document results of patient/resident care within established policies,proceduresand scope of practice.
• Nutritional care

• Completes all other duties as assigned.

Qualifications:

Required:

• Must be in good standing on the State Nursing Assistant registry orappropriate entity

Preferred:

• Previoushospital or long-termcare or skilled nursing experience
• CPR Certified
