Qualifications

• Minimum of three (3) years of nursing experience in an acute care setting.
• Licensed Registered Nurse in the State of Virginia or eligible.
• Current RN licensure in Virginia; complete 15 Continuing Education Units per year.
• Master's Degree in Nursing or a healthcare related field from an accredited program (preferred).

Responsibilities

• Works with the attending and consulting physicians to facilitate effective and efficient transition through the hospital stay.
• Collaborates with the multi-disciplinary team to ensure patient needs are met and care is coordinated across the continuum, and that resources are efficiently utilized.
• Seeks social work expertise to resolve psychosocial issues and develop complex discharge plans; interacts with patients, families, professionals, and agencies.
• Serves as a liaison between the hospital and community agencies for exchange of information.
• Maintains compliance with Quality Improvement Organization (QIO) series and CMS guidelines; participates in patient safety initiatives.

Baccalaureate Degree in Nursing

Master's Degree in Nursing or a healthcare related field
