Job summary

Rehab Without Walls Neuro Rehabilitation is seeking an experienced LCSW to provide counseling and support across home and community settings. The role focuses on addressing social, emotional, and functional goals within a multidisciplinary rehab team.

The candidate will conduct evaluations, set objectives with the team, and guide families in coping strategies while coordinating care and linking to community resources. A state LCSW license and BLS/CPR are required.

Qualifications

• Master's Degree in Clinical Social Work (LCSW) is required.
• Current Licensed Clinical Social Worker licensure in the state of practice.
• Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis.
• Current BLS First Aid and CPR certification required.

Responsibilities

• Performs comprehensive evaluations of patients and families.
• Sets measurable objectives with the rehab team.
• Assists health team in understanding social and emotional factors related to health problems.
• Instructs/counsels patients and families for coping with illness.
• Participates in care coordination and resource identification for patient needs.
• Identifies and connects patients/families with community resources to meet goals.
