Job summary

ReMed, a Collage Rehabilitation Partner, in Pittsburgh, PA, seeks a dedicated Rehabilitation Case Manager to oversee client treatment plans from admission through discharge. You will serve as the central contact for clients, families, and providers, coordinating therapy schedules and ensuring high-quality care.

The role emphasizes collaboration with the clinical team, documentation accuracy, and support for funding and compliance activities.

Qualifications

• Bachelor’s degree in healthcare, human services, or a related field required.
• Master’s degree preferred.
• Certified Rehabilitation Counselor (CRC) or Certified Case Manager (CCM) designation preferred.
• Hands-on case management experience within the healthcare industry.
• Valid driver’s license with a record of responsible driving, travel up to 30% of the time and ability to lift up to 50 lbs.

Responsibilities

• Lead interdisciplinary treatment team meetings to set, monitor, and revise rehabilitation goals.
• Meet regularly with clients to incorporate their personal goals into care plans and advocate for client needs.
• Collaborate with the Clinical Director, direct care staff, and external physicians to design strategies for comprehensive care plans.
• Maintain consistent contact with client families and support networks to deliver updates, education, and orientation.
• Monitor care programs according to funding guidelines and facilitate alternative/future funding sources; complete required documentation for billing integrity.
• Maintain thorough, timely, and accurate client charts in accordance with state regulations and HIPAA/HITECH guidelines.
• Assist with clinical data collection for company-wide quality assurance and outcomes initiatives.
