Job summary

Collage Rehabilitation Partners in Gilroy, CA is seeking a Case Manager to oversee active rehabilitation caseloads across residential and day programs. You will coordinate treatment goals with the team, ensure plans are followed from admission through discharge, and collaborate with physicians and external providers to support client care.

Strong case management background in rehab is required, with a valid driver’s license.

Qualifications

• Master’s degree or licensed clinician, including an RN.
• Case management experience in healthcare/rehabilitation.
• Valid driver's license required.

Responsibilities

• Manage all case provisions for active rehabilitation caseload.
• Lead team meetings and coordinate goals and discharge plans.
• Ensure implementation of treatment plans from admission to discharge.
• Coordinate with physicians and other health care providers.
• Schedule therapies and appointments and communicate with families.
• Maintain internal reports and chart documentation.
• Assist with funding issues and advocate for clients.
• Support Program Director with onboarding training and performance.
