ReMed, a Collage Rehabilitation Partner, seeks a dedicated Rehabilitation Case Manager to oversee client treatment plans from admission to discharge in Pittsburgh, PA. You will coordinate therapy schedules, lead interdisciplinary team meetings, and maintain compliant clinical documentation to ensure high-quality care.

The role requires a Bachelor's in healthcare or related field (Master's preferred), and hands-on case management experience. Travel up to 30% and lifting up to 50 lbs are required.

Qualifications

• Bachelor's degree in healthcare, human services, or related field required.
• Master's degree preferred.
• CRC or CCM designation preferred.
• Hands-on case management experience in healthcare.
• Valid driver's license with travel up to 30% and ability to lift up to 50 lbs.

Responsabilités

• Lead interdisciplinary treatment team meetings to set, monitor, and revise rehabilitation goals.
• Meet with clients to incorporate personal goals into care plans and provide guidance across care phases.
• Collaborate with Clinical Director, direct care staff, and external physicians on care plans.
• Maintain contact with client families and support networks for updates and education.
• Monitor funding guidelines and facilitate alternative funding sources; complete required documentation.
