Qualifications

• Bachelor's degree in healthcare, human services, or a related field required; Master's degree preferred.
• Certified Rehabilitation Counselor (CRC) or Certified Case Manager (CCM) designation preferred.
• Hands-on case management experience in healthcare; crisis management, needs assessment, and treatment plan formulation.
• Valid driver's license with travel up to 30% and ability to lift up to 50 lbs.

Responsibilities

• Lead interdisciplinary treatment team meetings to set, monitor, and revise rehabilitation goals.
• Meet regularly with clients to incorporate personal goals into care plans; advocate for client needs and provide guidance.
• Work with Clinical Director, direct care staff, and external physicians to design strategies for care plans.
• Maintain contact with client families and support networks to deliver updates, education, and orientation.
• Monitor funding guidelines, facilitate alternative funding sources, and complete required documentation for billing integrity.
• Maintain thorough, timely client charts and log notes per state regulations and HIPAA/HITECH guidelines.
• Assist with clinical data collection for quality assurance and outcomes initiatives.
