Qualifications

• Must possess or be eligible for a New York State RN license.
• Bachelor’s Degree in Nursing preferred with acute care experience.
• ACLS and BLS certifications preferred.

Responsibilities

• Assess patient condition and coordinate nursing care with other clinicians.
• Develop individualized care plans including teaching and discharge planning.
• Administer direct patient care and medications as ordered.
• Practice infection control and maintain a safe environment.
• Document changes in patient status and communicate with RN/Clinician.
• Maintain and update written care plans and set patient/family goals.
• Educate patient/family and ensure alignment with physician care plans.
• Collaborate with staff and departments to improve patient care.
• Participate in unit or hospital quality assurance activities.
