Cleveland Clinic is expanding its Neuro Institute and seeks a Case Management Social Worker to join the Nursing Institute on the Main Campus. You will collaborate with multidisciplinary teams to coordinate patient transitions and provide compassionate, patient-centered support for complex neurological cases.

Responsibilities include crisis intervention, care coordination, discharge planning, and community resource navigation.

Formación

• Master's Degree in Social Work or MSSA required.
• Current state licensure as a Social Worker (LSW/LISW/LISW-S/PC).
• BLS certification through AHA or Red Cross.
• Minimum 3 years of healthcare system experience.

Responsabilidades

• Coordinate discharge planning for assigned patients to ensure timely transitions.
• Communicate with interdisciplinary teams about discharge status.
• Educate patients and families on post-acute options and costs.
• Conduct psychosocial assessments and interventions to support adherence to treatment plans.
• Advocate for patients and families on care decisions and access to services.
• Provide counseling and support for end-of-life decisions when needed.
• Stay current with regulatory requirements and mandatory education.
• Support department goals and professional growth.
