Job summary

Cleveland Clinic Neurological Institute is seeking a Case Management Social Worker to coordinate patient transitions and provide psychosocial support for patients with complex medical needs. Embedded within neuro units and ICU, you will collaborate with nursing and medical teams to optimize care planning and patient outcomes.

You will conduct psychosocial assessments, discharge planning, and resource navigation, empowering patients and families to access community resources and self-manage care.

Qualifications

• Master's Degree in Social Work or MSSA required.
• Current state social work licensure (LSW/LISW/LISW-S/PC) required.
• BLS certification (AHA or Red Cross) required.

Responsibilities

• Coordinate discharge planning for assigned patients to ensure timely transitions.
• Communicate discharge status with interdisciplinary care teams.
• Educate patients and families about post-acute options and finances.
• Perform psychosocial assessments and interventions to support treatment adherence.
• Provide counseling and advocacy for patient and family decision-making and safety.

Education

Master's Degree in Social Work

Current state licensure as a Social Worker (LSW, LISW, LISW-S or PC)

Basic Life Support (BLS) certification
