You will serve as a key resource across inpatient units, performing psychosocial assessments and coordinating discharge planning.

As a Case Management Social Worker, you’ll collaborate with interdisciplinary teams, support post-acute options, and ensure timely discharges while addressing social barriers and family needs.

Formación

• Master’s Degree in Social Work or MSSA is required.
• Current state licensure as a Social Worker (LSW/LISW/LISW-S/PC) is required.
• BLS certification through AHA or Red Cross is required.
• Minimum of 3 years of experience in healthcare systems.

Responsabilidades

• Manage all aspects of discharge planning for assigned patients to ensure a timely discharge and appropriate post‑discharge connections.
• Communicate with interdisciplinary team about discharge planning status for all referred patients.
• Educate patients, families and physicians regarding post‑acute options and address financial concerns.
• Conduct initial and ongoing psychosocial assessments and interventions to engage patients and families in adhering to the treatment plan.
• Provide counseling and intervention on treatment decisions and end‑of‑life issues.
• Advocate for patients and families in cases involving abuse and neglect, domestic violence, elderly or institutional abuse and sexual assault.
• Promote patient and family empowerment to make autonomous health care decisions and access needed services.
• Meet core social work competencies and adhere to regulations.
• Remain current with regulations impacting patient care and regulatory compliance.
• Promote professional growth and meet education requirements.
• Support department goals contributing to organizational success.

Educación

Master’s Degree in Social Work or MSSA

Current state licensure as a Social Worker (LSW, LISW, LISW-S or PC)

Basic Life Support (BLS) through AHA or American Red Cross

3 years healthcare experience
