Rockford Nephrology Associates is seeking a motivated Advanced Practice Provider to join our nephrology team in Rockford, IL. The role offers care across office, hospital, and dialysis settings with no weekends and no call, and a starting salary of $130,000+ depending on experience.

Qualifications

• Master's or doctoral degree from an accredited NP/PA program.
• Current Illinois NP/PA license and national board certification.
• Experience in nephrology, dialysis, or hospital medicine preferred, but motivated NPs may apply.

Responsibilities

• Provide comprehensive kidney care across office, hospital, and dialysis settings.
• Evaluate and manage patients with chronic kidney disease and related conditions.
• Perform histories and physical examinations, develop treatment plans.
• Order and interpret labs and imaging, coordinate referrals.
• Educate patients and families on kidney health and disease management.
