Job summary

Rockford Nephrology Associates (RNA) is seeking an Advanced Practice Provider to join a collaborative nephrology team in Rockford, IL. This role offers no weekends and no call, broad exposure across outpatient, hospital, and dialysis settings, and a starting salary of $130,000+ with CME allowance and generous PTO.

Ideal candidates have NP/PA licensure in Illinois, board certification, and strong clinical skills with a patient-centered approach.

Qualifications

• Master's or doctoral NP/PA degree from an accredited program.
• Current Illinois NP/PA license and national board certification.
• Strong clinical assessment, diagnostic, and treatment-planning skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with physicians and multidisciplinary teams.
• Commitment to providing compassionate, patient-centered care.
• Nephrology, dialysis, hospital medicine, or related experience preferred but applicants motivated to develop nephrology expertise are encouraged.

Responsibilities

• Provide comprehensive care to patients with kidney disease across multiple clinical settings.
• Evaluate and manage CKD and related conditions.
• Perform histories and physical examinations.
• Develop and monitor treatment plans.
• Coordinate care between the dialysis center, outpatient clinic, and hospital.
• Educate patients and families about kidney health and treatment options.
• Discharge planning and follow-up planning.
