Job summary

Rockford Nephrology Associates seeks an experienced Advanced Practice Provider to deliver comprehensive nephrology care across office, hospital, and dialysis settings. Enjoy a predictable schedule with no weekends and no call, while earning a strong starting salary and CME benefits.

You'll work with an established nephrology team, expanding your expertise in kidney disease, dialysis, inpatient management, and chronic disease care in a supportive environment.

Qualifications

• Master's or doctoral degree from an accredited NP program or PA program.
• Current Illinois NP/PA license and national board certification.
• Nephrology experience preferred, but motivated APPs are encouraged to apply.

Responsibilities

• Assess and manage chronic kidney disease in outpatient nephrology
• Perform H&P and develop individualized treatment plans
• Order and interpret labs and imaging
• Coordinate referrals and care across dialysis, hospital, and clinic
• Educate patients on kidney health and treatment options
• Assist in discharge planning and post-hospital follow-up
• Provide comprehensive care in dialysis and inpatient nephrology settings
