Job summary

Rockford Nephrology Associates (RNA) is seeking an Advanced Practice Provider to join a collaborative nephrology team with care across office, hospital, and dialysis settings. This role offers guaranteed weekday scheduling, no weekends, and no on-call obligations, with a strong focus on comprehensive patient care.

You’ll work closely with physicians to deliver care, with opportunities to grow nephrology expertise in a supportive environment and participate in varied clinical settings including

Qualifications

• Master's or doctoral degree from an accredited NP/PA program.
• Current Illinois NP/PA license and national board certification.
• Strong clinical assessment, diagnostic, and treatment-planning skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with physicians and multidisciplinary teams.
• Commitment to providing compassionate, patient-centered care.
• Nephrology, dialysis, hospital medicine, or related experience preferred, but motivated NPs/PAs are encouraged to apply.

Responsibilities

• Evaluate and manage patients with CKD and related conditions in outpatient nephrology.
• Perform comprehensive histories and physical examinations.
• Develop individualized treatment plans and monitor progress.
• Order and interpret laboratory and diagnostic studies.
• Manage medications and coordinate referrals.
• Educate patients on kidney health, disease management, and prevention.
• Support patients in long-term health management.
