Rockford Nephrology Associates (RNA) in Rockford, Illinois is expanding its nephrology care team and inviting an Advanced Practice Provider to join a collaborative group. The role covers outpatient nephrology, inpatient hospital care, and dialysis settings, with a focus on comprehensive, patient-centered kidney care.

Qualifications

• Master's or doctoral degree from an accredited NP/PA program.
• Current Illinois NP/PA license and national board certification
• Strong clinical assessment, diagnostic, and treatment-planning skills
• Excellent communication and interpersonal skills
• Ability to work collaboratively with physicians and multidisciplinary teams
• Commitment to providing compassionate, patient-centered care
• Nephrology, dialysis, hospital medicine, or related experience preferred; willingness to develop nephrology expertise

Responsibilities

• Provide comprehensive care to patients with kidney disease in outpatient, hospital, and dialysis settings.
• Evaluate and manage CKD and related conditions; perform histories and physical examinations.
• Develop individualized treatment plans and monitor patient progress.
• Order and interpret laboratory and diagnostic studies; coordinate referrals.
• Educate patients on kidney health and disease management; promote prevention.
• Manage dialysis care, ESRD complications, and vascular access concerns.
• Participate in inpatient nephrology management and discharge planning.
• Collaborate with the nephrology team to ensure continuity of care.
