Job summary

Rockford Nephrology Associates (RNA) is seeking an Advanced Practice Provider to join a collaborative nephrology team. This role offers care across outpatient, hospital, and dialysis settings with no weekends and no call, delivering predictable work-life balance.

Starting salary is $130,000+ with CME allowance and generous benefits. You’ll gain exposure to kidney disease management and dialysis, with opportunities to grow in a supportive environment.

Qualifications

• Master’s or doctoral NP/PA program degree required.
• Illinois NP/PA license and national board certification.
• Strong clinical assessment, diagnostic, and treatment-planning skills.
• Excellent communication and interpersonal skills.
• Ability to work with physicians and multidisciplinary teams.
• Compassionate, patient-centered care.
• Nephrology, dialysis, hospital medicine, or related experience preferred.

Responsibilities

• Evaluate and manage CKD patients and related conditions.
• Perform comprehensive histories and physical examinations.
• Develop and monitor individualized treatment plans.
• Order and interpret labs and diagnostic studies.
• Educate patients on kidney health and disease management.
• Coordinate care between dialysis, clinic, and hospital teams.
• Support discharge planning and follow-up after hospitalization.
