Rockford Nephrology Associates (RNA) is seeking a motivated Advanced Practice Provider to join our nephrology team. This opportunity spans office, hospital, and dialysis settings, offering a balanced schedule with no weekends and no call.

The ideal candidate will have an NP/PA certification and a strong interest in nephrology, with collaborative support from an experienced team. Experience in nephrology, dialysis, or hospital medicine is preferred, and RNA supports professional development with

Qualifications

• Master's or doctoral degree in NP/PA program required.
• Current Illinois NP/PA license and national board certification.
• Strong clinical assessment, diagnostic, and treatment‑planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively with physicians and multidisciplinary teams.
• Nephrology, dialysis, hospital medicine, or related experience preferred but not required.

Responsibilities

• Evaluate and manage patients with kidney disease in outpatient nephrology, dialysis, and hospital settings.
• Perform comprehensive histories and physical examinations.
• Develop individualized treatment plans and monitor patient progress.
• Order and interpret labs and diagnostic studies.
• Coordinate care with nephrology team and other providers.
• Educate patients and families on kidney health and disease management.
• Support long‑term patient health and disease prevention.
