Job summary

Essentia Health’s Superior Clinic nephrology team seeks a Nurse Practitioner or Physician Assistant to provide patient-centered nephrology care. You’ll collaborate with physicians and other clinicians, support dialysis rounding, and manage care across clinic, hospital, and dialysis settings in the Northwestern Wisconsin region.

Ideal candidates bring experience in complex chronic disease management, strong teamwork, and commitment to quality incentives.

Qualifications

• Experience in complex chronic disease management.
• Team-based care approach in dialysis and nephrology settings.
• Good communication with physicians and patients.

Responsibilities

• Assist physicians with non-encounter work and care coordination.
• Lead dialysis rounding with a patient-centric approach.
• Provide care independently and in collaboration with the nephrology team.
• Support clinic and hospital workflows as part of a 3-phase nephrology program.
