Job summary

Sanford Health in Eau Claire, WI, is seeking a Nephrology Nurse Practitioner to provide advanced care for patients with acute and chronic kidney diseases, including those on dialysis and post-transplant. You will collaborate with nephrologists and the interdisciplinary team to assess, test, treat, and educate patients and families.

Off-site dialysis center coverage in Eau Claire and Rice Lake, with hours Monday–Thursday (4 eight-hour days).

Qualifications

• Master’s or Doctorate degree from an accredited Nurse Practitioner program.
• If APRN was licensed prior to August 1, 1995, master’s degree in nursing is preferred.
• Nephrology Or Internal Medicine Experience Preferred.
• Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice.
• Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances in each state of practice.
• State Controlled Substance License (if required by State)
• National board certification as an Adult Gerontology Acute Care Nurse Practitioner (AGACNP-BC) or ACNPC-AG), Acute Care Adult (ACNP-BC)
• Obtains and subsequently maintains required specific competencies and certifications as outlined on privilege form.

Responsibilities

• The NP provides advanced nursing care and management to patients with acute and chronic kidney diseases, including those on dialysis and post-transplant.
• Works with nephrologists and the interdisciplinary team to perform assessments, order and interpret diagnostic tests, develop and implement treatment plans, and educate patients and their families.
• Educates patients and families on disease processes, medications, treatment options, lifestyle modifications, and transplant considerations.
• Monitors and adjusts care plans for dialysis patients in inpatient and outpatient settings.
• Documents all patient interactions and clinical decisions in the electronic health record.
• Collaborates closely with nephrologists, dialysis teams, dietitians, social workers, and other healthcare professionals.
