Qualifications

  • Bachelor's Degree
  • Physical Therapy
  • Licensed Physical Therapist (PT) – IDFPR
  • Basic Life Support (BLS) within 30 days – AHA
  • Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients

Responsibilities

  • Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor; uses safe body mechanics.
  • Educates patients, family, caregivers, students and other health care providers.
  • Designs and adjusts a plan of care based on patient response and evidence; documents in CTSCP67.
  • Evaluates clinical findings to determine PT diagnoses and outcomes of care.
  • Supervises PTA and rehab technicians; communicates status and discharge recommendations.

Education

Bachelor's Degree
Physical Therapy
Licensed Physical Therapist (PT) – Illinois Department of Financial and Professional Regulation (IDFPR)
Basic Life Support (BLS) within 30 days – American Heart Association (AHA)
