Qualifications

• Licensed Physical Therapist (PT) in IL.
• BLS certification within 30 days.
• Completion of required CE for acute stroke care.

Responsibilities

• Designs an appropriate plan of care integrating goals, treatment, outcomes and discharge plans.
• Educates patients, families, caregivers and students using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examination with reliable methods related to chief complaint and history.
• Produces documentation per CTSCP67 and CMS/regulatory standards.
• Supervises PTA and rehab technicians and communicates progress to the medical team.
