Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care within scope of practice, ensuring patient needs are met in accordance with standards, physician orders, and center policies.

The role requires long-term care experience, PCC charting proficiency, and a valid California LVN/LPN license with BLS/CPR certification. Nursing management experience is preferred but not required.

Qualifications

• Licensed LVN/LPN in the state of California.
• Experience in long-term care setting.
• Proficiency with Point Click Care charting.
• Current BLS/CPR certification.
• Nursing management experience preferred but not required.

Responsibilities

• Lead day-to-day nursing assistants and ensure compliance with policies.
• Manage admissions, transfers, and discharges.
• Document changes in patient conditions and notify departments.
• Perform rounds with physicians as needed.
• Maintain narcotics counts, pharmacy orders, and restraint reduction.
• Adhere to incident reporting procedures.
• Ensure charting accuracy and program monitoring per Quality Life Manual.
• Document room changes, LOA, new admits, or discharges.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Lead the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines
• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established
• Admit, transfer, and discharge residents as necessary
• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.

Makes rounds with physicians when necessary.
