Sweetwater Care in Dinuba, CA seeks a qualified LVN/LPN to provide nursing care within scope of practice in accordance with standards and physician orders. This full-time role emphasizes patient assessment, documentation, and collaboration with interdisciplinary teams.

Required LTC experience, PointClickCare charting experience, active LVN/LPN license in California, and BLS/CPR certification. Nursing management experience is a plus but not required; we value reliability and compassionate patient

Responsabilités

• Follow established procedures for charting and incident reporting.
• Follow procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions (nutrition, infections, skin conditions, weight loss, etc.).
• Utilize the Quality Life Manual to identify, place, and monitor patients in relevant programs.
• Document and notify departments of patient room changes, LOA, new admits, or discharges.
• Make rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
