Job summary

Sweetwater Care in Willows, CA is seeking a licensed LVN/LPN to provide nursing care within scope of practice, ensuring patients' needs are met per standards, orders, and policy.

The role emphasizes meticulous charting, patient condition monitoring, and collaboration with physicians. LTC experience and PCC charting proficiency are highly valued; BLS/CPR certification is required or willingness to obtain it.

Qualifications

• Must have long-term care experience in a clinical setting.
• Experience with Point Click Care charting.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
