Job summary

Sweetwater Care in Chowchilla, CA is seeking an LVN/LPN to provide nursing care within scope of practice, in accordance with standards of practice, physician orders and center policies.

The successful candidate will document patient changes, participate in rounds with physicians, assist with charting, narcotic counting and restraint reduction, and follow established procedures to ensure high-quality patient care while complying with federal, state and local guidelines.

Qualifications

• Must have long term care experience.
• Experience with PointClickCare charting.
• Active LVN/LPN license in California.
• BLS/CPR certification or willingness to obtain.
• Nursing management experience preferred but not required.

Responsibilities

• Follow established charting and incident reporting procedures.
• Follow procedures for narcotics counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions, including nutrition, infections, skin issues, and weight.
• Use the Quality Life Manual for patient identification, placement, and monitoring.
• Document and notify departments of room changes, LOA, admissions, or discharges.
• Make rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
