Job summary

Sweetwater Care in Clovis, CA is seeking a dedicated LVN/LPN to provide nursing care within scope of practice, following standards of care, physician orders, and center policies. Qualifications

• Long-term care experience is required.
• Experience with PointClickCare charting.
• Active LVN/LPN license in California.
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience is preferred but not required.

Responsibilities

• Lead day-to-day functions of the nursing assistants per policies.
• Ensure nursing staff comply with policies and procedures.
• Admit, transfer, and discharge residents as needed.
• Follow charting and incident reporting procedures.
• Follow narcotics counting, pharmacy orders, and restraint reduction procedures.
• Document changes in patient conditions (nutrition, infections, skin issues, weight loss, etc.).
• Utilize the Quality Life Manual for patient placement and monitoring.
• Notify applicable departments of patient room changes, LOA, admissions, or discharges.
• M ake rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Lead the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines
• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established
• Admit, transfer, and discharge residents as necessary
• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.

Makes rounds with physicians when necessary.
