Job summary

The Oaks II seeks a health navigator to connect CareLink members with clinics and providers, improving health education and appointment adherence. You will guide patients through access to care, education, and care coordination, bridging to CareLink funding as needed and following up to ensure ongoing health needs are met.

Essential duties include establishing rapport with patients, entering data into the EMR, and reporting to the RN Case Manager.

Qualifications

• Bachelor’s degree in social sciences, health education or related field is required.
• Four years of experience in health education, public health, social work or case management may be considered in lieu of the degree.
• Community Health Worker certification preferred.
• English/Spanish fluency is required.

POSITION SUMMARY/RESPONSIBILITIES

This position will serve as the liaison between our CareLink members, clinics, and providers to facilitate improved health education, appointment compliance, along with appropriate utilization of services e.g., reduction of unnecessary visits for emergency services. This position will assist and guide patients with access to care, education, support, and patient care coordination. Patients will be navigated to medical appointments, re-connected to CareLink for funding (if applicable) and followed up with health care needs. All functions and behaviors are applicable to the clinical and community settings. Additional activities will include establishing rapport, data entry into the electronic medical record (EMR) and reporting to RN Case Manager.

EDUCATION/EXPERIENCE

Bachelor’s degree in social sciences, health education or related field of study is required. Four years of experience in the area of health education, public health, social work and/or case management may be considered in lieu of the Bachelor’s degree. Community Health Worker certification preferred. Knowledge and experience working with programs of similar scope and focus preferred. English/Spanish fluency is required.
