Gold Coast Health Plan is seeking a Health Navigator to implement and coordinate a Health Navigator Program under the Health Education, Cultural and Linguistic Services division. The navigator will guide members through the healthcare system, support them in establishing a medical home, and assist in reducing barriers to care.
The role includes post-ER contact, scheduling follow-ups with PCPs, coordination with hospitals and clinics, and education on appropriate use of health services.
Under the direction of the Director of Health Education, Cultural and Linguistic Services, and Disease Management (Director of Health Education) this person will assist in the implementation and coordination of a Health Navigator Program. The Health Navigator services may include, but not limited to, guiding members through the healthcare system, help members establish a “medical home” and assist members in reducing barriers to obtaining their health care needs. The Health Navigator will establish contact with members who utilize the emergency room and assist members in reconnecting with their primary care provide after an emergency room encounter.
Education: Possession of a baccalaureate degree (BA) from an accredited college or university with a major in: community health promotion, health education, sociology, psychology/health psychology or closely related field. Experience: Experience in working with low-income, diverse populations. Experience in working in the healthcare field including hospitals. Other: Other medical training or experience and other certification may be evaluated on a case by case basis. Knowledge, Skills, and Abilities: Knowledge of Manage Care and health care principles; knowledge of project planning and evaluation techniques; strong understanding of cultural needs of diverse ethnic minority populations and sensitivity to community and cultural groups; ability to related to other professionals and ability to prepare reports and correspondence.
