Job summary

Yellowhawk Tribal Health Center seeks a CD Prevention Coordinator to design and implement a comprehensive prevention plan for the CTUIR community, aligned with Yellowhawk Mission and Vision.

You will collaborate across Yellowhawk programs, schools, and state partners; lead prevention activities, manage a small youth caseload, and deliver evidence-based prevention strategies to reduce substance use and strengthen resilience in youth and families.

Qualifications

• High School Diploma or GED required.
• Active MHACBO CPS certification or ability to obtain within 2 years of hire.
• Active Oregon THW certification or ability to obtain within six months of employment.
• Proven knowledge of substance use disorders and lived experience or prior SUD employment.
• Experience with event planning and collaborating with multiple departments/agencies.
• Letter of Verification proving minimum 2 years sobriety for those in recovery.
• Food Handler’s Card and CPR/First Aid certifications within 3 months.
• Represents Yellowhawk with a positive, alcohol- and drug-free lifestyle (including social media).
• Demonstrated computer skills with Microsoft Office and office equipment.
• Demonstrated verbal and written communication skills.
• Knowledge of history, culture, and health needs of AI/AN communities.
• Valid driver’s license and driving/insurance for GSA vehicles.
• Must pass background check and pre-employment drug screening.

Responsibilities

• Collaborate with Yellowhawk programs, CTUIR departments, and county/state partners.
• Develop a comprehensive prevention plan and curriculum for the CTUIR community and schools.
• Lead, co-facilitate, and organize prevention activities involving large groups of youth.
• Manage a small caseload of at-risk youth and provide targeted, developmentally appropriate support.
• Coordinate with schools and Yellowhawk programs to support students and families.
• Ensure prevention activities use evidence-based or Tribal Best Practice approaches.
• Attend planning meetings for committees as directed.
• Deliver presentations to schools, Health Commission, Youth Council, and community groups.
• Work with regional/state agencies to collect, analyze, and interpret local data.
• Collaborate with TPEP, Family Strength, and other prevention programs.
• Perform other duties as assigned.
