Job summary

AMEDISYS HOME HEALTH seeks a certified music therapist to provide hospice music therapy services to patients in need of comfort and support. You will develop and coordinate an organized music therapy program, tailor plans of care, and document in accordance with regulatory and accreditation requirements.

A licensed driver with a reliable vehicle is required for home visits. You will participate in quality assessment activities and collaborate with a multidisciplinary team to improve patient

Qualifications

• Certified music therapist from an established music therapy institute.
• Licensed driver with reliable automobile which is insured in accordance with state law.

Responsibilities

• Develops, coordinates and provides an organized music therapy program.
• Provides music therapy in alignment with the needs of the patient and physical limitations.
• Develops a plan of care.
• Completes all documentation as required by the organization in compliance with legal, governmental and accreditation requirements.
• Participates in quality assessment and performance improvement activities.
• Performs other job-related tasks as requested.
