Job summary

The candidate must be a certified music therapist with a state-licensed driver and reliable vehicle to meet patient visit requirements. This position offers flexible hours as needed to support patient care across the hospice program.

Qualifications

• Certified music therapist from an established music therapy institute.
• Licensed driver with reliable automobile insured as per state law.

Responsibilities

• Develops, coordinates and provides an organized music therapy program.
• Provides music therapy aligned with patient needs and physical limitations.
• Develops a plan of care.
• Completes all documentation as required by the organization in compliance with legal, governmental and accreditation requirements.
• Participates in quality assessment and performance improvement activities.
• Performs other job-related tasks as requested.
