Job summary

Amedisys Home Health is seeking a qualified Music Therapist to join our hospice team on a PRN/Per Diem basis in Bellevue, Wisconsin. You will assess patients, plan and provide organized music therapy interventions aligned with patient needs and allowable physical limitations, and document care according to regulatory standards.

The ideal candidate holds a certified music therapist credential, a valid driver’s license and reliable vehicle.

Qualifications

• Certified music therapist from an established music therapy institute.
• Licensed driver with reliable automobile which is insured in accordance with state law.

Responsibilities

• Develops, coordinates and provides an organized music therapy program.
• Provides music therapy in alignment with the needs of the patient and physical limitations.
• Develops a plan of care.
• Completes all documentation as required by the organization in compliance with legal, governmental and accreditation requirements.
• Participates in quality assessment and performance improvement activities.
• Performs other job-related tasks as requested.
