Job summary

Advocate Health seeks a licensed Physical Therapist to join its Charlotte float pool, supporting Carolinas Rehabilitation facilities and related inpatient and acute care sites. The role emphasizes comprehensive patient evaluation, planning, and crossover coverage.

You will collaborate with a multidisciplinary team, educate patients and caregivers, and ensure compliant documentation and billing. A DPT is preferred, with strong clinical judgment and communication skills essential.

Qualifications

• Must hold an active Physical Therapy license in the employment states.
• Degree in Physical Therapy from an accredited program; DPT preferred or equivalent per graduation date.

Responsibilities

• Perform patient evaluations and develop care plans per PT practice act.
• Collaborate with clinicians to coordinate scheduling across sites.
• Educate patients and caregivers on diagnosis, home programs, and equipment.
• Document evaluations and treatment in patient records and billing system.
• Ensure adherence to HIPAA and department policies.
• Maintain department cleanliness and assist with coverage across sites.
