Job summary

Advocate Health is seeking a Physical Therapist to join the outpatient rehabilitation team in the Charlotte area. This full-time role supports a regional float across Northeastern Greater Charlotte sites, including University City and Kannapolis, requiring pelvic PT experience and a state-licensed PT.

The position emphasizes comprehensive patient evaluation, development of individualized plans, and collaboration with physicians and staff to ensure quality care.

Qualifications

• Bachelor's degree in Physical Therapy program required.
• Applicable state license for Physical Therapy required.
• BLS required per policy guidelines.

Responsibilities

• Evaluates patients and establishes a care plan including modalities, exercises, and education.
• Gathers data from medical records and family to inform treatment.
• Sets functional goals with patients and provides evidence-based interventions with ongoing assessment.
• Documents evaluations, tests, progress notes, and charges as needed.
• Communicates progress to physicians and coordinates patient schedules and discharge planning.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
